CRITERIA
• Patient MUST be diabetic
• Patient MUST check blood glucose levels at least once per day
• Patient MUST take insulin at least once per day
• Patient MUST have an active Medicare Part B plan
• Patient MUST NOT have received a CGM or refills in the last 3 years
• No age limit
